- AXG Referral Form 2022
- Shipping to and from your home.
Service Requirements:
- Completed AXG Sleep Diagnostics LLC referral form.
- Copy of photo identification of the person utilizing the service
- Completed forms: Service Contract, HIPPA, Credit Card Authorization (emailed at a later date)
- Ability to attach electrodes for the sleep study with detailed written and video instructions provided.
A comprehensive type 2 diagnostic home sleep study which records 7 channels as well as body position data. This DOES meet FAA requirements and includes:

- ECG/heart rate
- EMG
- Airflow
- Respiratory effort
- Oxygen saturation
- Body position
